Do You Use Information Collected by Phone, Fax or Mail?
This information will be used to communicate test results and
analysis and other account-related data as well as educational and promotional
information unless you indicate otherwise. You have the opportunity to opt-out
of any email communication by clicking on “Unsubscribe,” or you may request
that your account be “earmarked” for exclusion from any of our mailings.

Protection
To maintain POLARIS Laboratories’ ISO 17025 A2LA
accreditation, our Quality System mandates the execution of a written procedure
for maintaining, protecting and updating customer records and data. Therefore,
customer account information maintained within POLARIS’ Laboratory Information
Management System (hereafter referred to as LIMS) is protected by anti-virus
software that is installed on all servers, workstations and personal computers
within the POLARIS Laboratories domain. This software performs hourly downloads
to assure current file definitions are in place for virus and worm protection.
POLARIS servers and routers contain firewalls that ensure protection of the
data and records area of these systems. These firewalls are accessible through
username and domain name passwords. Servers storing critical files are checked
for virus infection nightly.
System
Maintenance
Manual system validation and disk integrity checks are run
every 45 days by authorized POLARIS Information Systems personnel. Operating
system and specific software upgrades are checked and monitored during system
validation. Data integrity and consistency are verified weekly through a
restoration of backups to ensure full data retrieval and restoration. Visual
server, workstation and PC inspections are performed routinely to confirm
optimal system functionality and performance and to eliminate file and/or data
corruption.
Data
Security
Protected partitions exist on all servers, workstations and
personal computers. For auditing purposes, all POLARIS personnel connected to
the LIMS and domain servers are given privileged access by unique usernames and
passwords according to organizational position. Personnel are granted access
only to those areas needed to perform daily job functions. Administrative
accounts are limited to authorized system administrators, the Chief Information
Officer and the Chief Science Officer exclusively.
